Membership Fee
A one time enrollment fee is due at the time the contract is signed.
$125/person ages 19 and above
$75/person ages 0-18

Maximum family monthly membership fee $325/month
Membership fees will be due on the first of every month. The first installment is due upon scheduling your first appointment and will be prorated for the month.
Automatic payment can be set up with a credit/debit card or bank account routing number. I will also accept cash or check.
